Informed Consent for Use of Residual Specimens for Quality Control

I authorize Kihealth Inc. to retain and use any remaining portion of my specimen, after completion of my ordered testing and required retention, for internal quality control and quality assurance activities to verify the accuracy, reliability, and consistency of laboratory testing.

No additional specimen collection will be performed for these purposes. My specimen and associated information will be handled confidentially in accordance with applicable privacy laws and laboratory policies. This consent does not authorize research or unrelated testing. Quality control activities are not intended to provide additional diagnostic results, and I will not be charged for these activities.

My consent is voluntary. Declining will not affect my care or access to ordered testing. I may withdraw consent for future use by contacting Kihealth Inc.; withdrawal cannot reverse uses already completed or permit retrieval of specimens that can no longer be linked to me. Specimens will be stored and disposed of according to applicable requirements and laboratory policies.
